How do they determine that an immune response is the same as the desired response they sought from the start? If they sell a drug with no expected outcome and then claim the outcome is expected, how can that be considered reasonable or safe for that matter.

Who ARE these people? The guy in the middle looks to be 75 or 80 or older, the guy asking the pertinent question looks like he is just now getting an inkling that something could be wrong. Bet he starts to worry!
